Home-Based Case Manager

JOB DESCRIPTION: 

Rising Stars’ Academy is a research-based social service agency established in 2008. Realizing numerous families are not acquiring the skills needed for self-sufficiency into adulthood, we have taken on the challenge of educating families with the skills needed to enhance their present living conditions and prepare them for their future endeavors. As a result, we offer services that serve as a catalyst to help shape the minds, nurture the souls, and impart values to children and their families. Services include home-based family centered casework and therapy, mentoring, parent education, tutoring/literacy, and visitation facilitation. 

SUMMARY OF POSITION: 

Rising Stars’ Academy is currently seeking a competent Home-Based Case Manager who is inspired by the opportunity to make a real difference in the lives of individuals. We are looking for a candidate who is empathetic, organized, and reliable. The candidate’s primary role will be advocating for clients while guiding them through the processes related to their cases. Home-Based Case Managers are responsible for identifying, monitoring, and evaluating the needs of clients. Additionally, they create and help implement treatment plans for clients’ so that clients’ needs can be met all while adhering to the mission and vision of Rising Stars’ Academy.
